Lincs Inspire’s exercise referral programme supports the Government’s putting patients first approach by providing access to tailored and supervised physical activity at the right time in the optimal care setting and, ultimately, reducing the overall burden on the health sector.

About Active Forever

At Lincs Inspire our exercise referral programme is designed to help patients take the necessary steps towards a lifestyle change that can lead to lower medication, less pain, and an increased quality of life.

Available to all residents over the age of 18 who are living with a health condition or undergoing rehabilitation. The specialist scheme leads to improved physical and mental wellbeing and helps patients to manage their symptoms, prepare or recover from surgery.

Members of the programme range in age, ability and fitness levels. Patients referred to us include those suffering from anxiety, hypertension, diabetes, obesity, back pain, angina, depression, osteoporosis, cancer, arthritis and other conditions.

Active Forever is a 12-week scheme with patients being referred by you, their health professional (i.e. practice nurses, health trainers, GPs or physiotherapists).

A short referral form needs to be completed by the health professional for each patient referred on to the programme.

The form seeks information from you on the patient’s medical condition so that we are fully informed before their referral programme commences.

Once a patient is referred by you our health improvement team will contact your patient to arrange an induction and a meet and greet.

We will then work with the patient to prescribe and deliver a suitable individualised programme, alongside a fitness assessment, and advice on active living.

Two activities are recommended per week, and we work closely with your patient to ensure these are suited to their abilities and needs.

Each member receives a review at 4 weeks, 8 weeks and upon completion of the programme.

Completing the programme is just the start of what we hope will become a regular active habit for your patients and we would love to see them continue to use our facilities and most importantly, stay healthy.

The Active Forever programme, once completed, will give patients the chance to sign up to a Lincs Inspire Full Health & Fitness membership at a special concessionary rate (if taken up within a month of completing the programme). After this month normal membership rates will apply.

Lincs Inspire is rooted in your local community. We are able to build a relationship of mutual trust, enabling us to link patients with other health and community services.
